Provocative mesenteric angiography for localizing ambiguous gastrointestinal hemorrhage

نویسندگان

چکیده

Objectives: Five percent of patients with recurrent gastrointestinal (GI) hemorrhage have indeterminate origin by radiological and endoscopic examinations. To improve diagnostic accuracy therapeutic embolization, the technique provocative mesenteric angiography (PMA) has been developed. It involves addition pharmacologic agents to standard angiographic protocols induce bleeding. Material Methods: This is an institutional review board-approved, retrospective study 20 who underwent PMA between 2014 2019. All had clinical evidence GI without a definite source. consisted anticoagulation 5000 units heparin selective transcatheter injection up 600 μg nitroglycerine, followed slow infusion 24 mg tissue plasminogen activator into arterial distribution highest suspicion artery. Results: Among PMA, 11/20 (55%) resulted in angiographically visible extravasation. Of these 11 patients, nine successful embolization coil or glue were discharged upon achieving hemodynamic stability. Two spontaneously stopped In our series, treatment 9/20 (45%) hemorrhage. No procedure-associated complications reported during procedure their course hospitalization. Conclusion: experience, effective safe approach localizing treating source bleeding about half otherwise unidentifiable

Mesenteric angiography for the localization and treatment of acute lower gastrointestinal bleeding.

BACKGROUND There is limited evidence regarding the effectiveness and complications of mesenteric angiography in the diagnosis and management of acute lower gastrointestinal bleeding (ALGIB). Our objective was to determine the complications and outcomes of mesenteric angiography in patients with ALGIB and to identify predictors of a positive result at angiography. METHODS We identified and rev...

Mesenteric angiography for acute gastrointestinal bleed: predictors of active extravasation and outcomes.

BACKGROUND Ongoing gastrointestinal bleeding (GIB) following endoscopic therapy and deciding between mesenteric angiography and surgery often challenge surgeons. We sought to identify predictors of positive angiographic study (active contrast medium extravasation) and characterize outcomes of embolization for acute GIB. METHODS We retrospectively analyzed angiographies for GIB at 2 teaching h...

Mesenteric panniculitis: still an ambiguous condition.

PURPOSE To study the possible relationship between mesenteric panniculitis (MP) visible on computed tomography (CT) and the presence of an underlying neoplastic disease. PATIENTS AND METHODS A retrospective analysis of 158 patients with CT examinations that revealed the presence of MP was performed. CT images were analyzed by two different radiologists using morphological criteria validated i...

The case for computed tomographic angiography for initial management of lower gastrointestinal hemorrhage.

Lower gastrointestinal hemorrhage remains a common disease, frequently presenting with acute life-threatening symptoms. Although prompt detection and treatment are imperative, it is difficult to diagnose lower gastrointestinal hemorrhage in an accurate and efficient manner. Most available modalities are time consuming. Computed tomographic angiography of the abdomen and pelvis, on the other han...

Diagnostic accuracy and implementation of computed tomography angiography for gastrointestinal hemorrhage according to clinical severity

OBJECTIVE This study compared the diagnostic accuracy of computed tomography (CT) angiography in patients with various severities of gastrointestinal hemorrhage (GIH). METHODS We retrospectively enrolled adult patients (n=262) with GIH who had undergone CT angiography from January 2012 to December 2013. Age, sex, comorbidities, presenting symptoms, initial vital signs, laboratory results, tra...
